What should the flow for Risk assessment? Actually I am working in production department of API Plant and I have to start Risk Assessment for new project / Product which has two intermediate stages and one API stages.

Identify the risks in the process, create a protocol and proceed.

Flow for Risk assessment
Identify the process, system for following sequence.

  1. Risk Identification
  2. Risk analysis
  3. Risk Evaluation
  4. Risk Control or reduce , IH Risk controlling phenomenon acceptance .
  5. Risk review
  6. Periodically risk evaluation If we propose as risk reduce phenomenon selected.
    Same manner you can follows sequence for API production process (es) stage.

My observation is that we are overdoing unwanted things but ignoring basic things. For example AHU internals completely SS304, normally not fabricated that way but because we overlook major things and try to mask by overdoing these things. Such actions will not mitigate any risks at all.
