If a format having effective date as 10.01.2019.
After 03 years, during the periodical review there are no changes in existing format, so review extension period stamp laid down on 10.01.2022, and next review date given as 10.01.2025.
Here my question is, if I want to prepare some document in August-2022 by using above said format. Then what is the format effective date to be mentioned on August document, either 10.01.2019 or 10.01.2022.
Plz, reply as early as possible.

It is not necessary to modify the format’s effective date unless there is a change in the format. SOP effective date may change, but format effective date will remain as such till it revised.
